The Effectiveness of Helicopter Based Intracranial Hemorrhage Transportation in Mountainous Area

Abstract
Objective
To investigate the effectiveness of helicopter emergency medical services (HEMS) in intracranial hemorrhage. We perform a review of neurosurgical patients who were transferred with HEMS in Gangwon province, Korea.
Methods
The retrospective study was performed with patients who were transferred by HEMS to our hospital, during two years. Among 364 patients, using HEMS, 15 patients of intracranial hemorrhage were included. Considering distance, 15 patients were included randomly in ground emergency medical service (GEMS) group. The effectiveness of HEMS in intracranial hemorrhage were evaluated according to the differences in the modified Rankin Scale (mRS), spent transfer time, initial lab data by GEMS and HEMS.
Results
The mRS at Postoperative 3 months was better using HEMS than using GEMS (p=0.013). The transfer time was faster in HEMS than GEMS (HEMS: 84.3±24.8 minute, GEMS: 128.6±62.1 minute) (p=0.011). Initial vital signs and lab data showed no statistical difference between HEMS and GEMS in Systolic Blood Pressure, Diastolic Blood Pressure, Arterial Blood Gas Analysis, O2 level, however, there is statistically significant between HEMS and GEMS in CO2 level in arterial blood gas analysis (HEMS: 33.2±3.0mmHg, GEMS: 37.7±6.6mmHg) (p=0.017).
Conclusion
Patients with intracranial hemorrhage have been shown to have better mRS using HEMS than GEMS, because of fast transfer time and success in intubation early in the course of intracranial hemorrhage. Helicopter Emergency Medical Services in Intracranial hemorrhage is effectiveness in specific condition, and more accurate triage with neurosurgeon for using HEMS should be considered.
