The Influence of Antiplatelet and/or Anticoagulant Agents on Recurrence of Chronic Subdural Hematoma |

Abstract |
Objective The use of antiplatelet agents and anticoagulants has increased in the elderly. The effects of these agents on the development and growth of chronic subdural hematoma (CSDH) are not clear. The purpose of this study was to analyze the clinical features of patients treated with antiplatelet agents and anticoagulants for the treatment of CSDH.
Methods A retrospective study was performed on 92 patients with CSDH from January 2010 to December 2012. All patients were underwent surgery with burr hole trephination and hematoma drainage. We divided the patients into three groups according to medication history (A: no medication, B: antiplatelet agents, and C: anticoagulants with/without antiplatelet agents). Trauma history, chief complaint on admission, bilaterality of hematoma, international normalized ratio (INR) prolongation, and recurrence rate were reviewed.
Results There were no statistically significant differences in trauma history, chief complaint on admission, and bilaterality of hematoma between the three groups. INR prolongation and recurrence more commonly occurred in the anticoagulant group compared with the no-medication group (p=0.003, 0.003).
Conclusion Use of anticoagulants is statistically associated with INR prolongation and increased recurrence. Also, the recurrence rate of CSDH was higher in the antiplatelet agent using group than in the no-medication group. Therefore, more caution and follow-up will be needed after surgery in patients given medication. |
