Predisposing Factors of Postoperative Infection after Craniotomy in Elderly Patients

Abstract
Objective
Postoperative infection after craniotomy is important complication that can lead to severe results in patients. It must be managed as soon as possible. So, this study investigated the variable factors associated with postoperative infection.
Methods
We retrospectively reviewed 189 patients above 60 year-old whom underwent craniotomy at our institution between January 2011 and December 2012. All patients were analyzed based on the information acquired from the various clinical factors(gender, age, past history, time of ambulation and drain tube maintenance, duration of operation and anesthesia, type of operation, CSF leakage and other organ infection), lab data (white blood cell count, erythrocyte sedimentation rate, C-reactive protein), and culture related factors (bacterial species drain tube culture, type of antibiotics). Then, we analyzed the relationship between each parameter and presence of postoperative infection.
Results
Among the 189 elderly patients whom underwent craniotomy, postoperative infection occurred in 9 patients (4.8%) while the rest of 180 patients (95.2%) had no postoperative infection. And out of theses variable factors, postoperative infection was associated with presence of previous craniotomy history (13.2%), drain tube culture positive (24.2%) and bacterial species of drain tube culture (MRSA) statistically significance in univariate analysis (p<0.05).
Conclusion
According to our study, patients with presence of previous craniotomy history and bacterial culture at a drain tube had a higher tendency of postoperative infection after craniotomy. This results were crucial information in planning the postoperative management in elderly patients.
