Associated Injury of Osteoporotic Compression Fractures: The Role of 99mTc-MDP Bone Scintigraphy

Abstract
Objective
Osteoporosis is common finding in geriatric patients. Osteoporotic vertebral fractures are two to three times more frequent than hip fractures, especially for women. Current operative methods include vertebroplasty and kyphoplasty, which significantly reduce pain and improve mobility of the patient and consequently, the quality of life. However, concerning too much to the spinal fracture, we sometimes failed to notice the associated injury. Here, authors showed the prevalence of the associated injury with osteoporotic compression fracture, and role of Tc-99m MDP bone scintigraphy.
Materials
From March 2005 to June 2007, 57 patients with osteoporotic compression fracture were treated by medical and surgical treatment. Among them, 39 patients were evaluated by Tc-99m MDP bone scintigraphy to find another bony lesion. We reviewed the whole body bone scan of 39 patients and analyzed the associated injury.
Results
Fourteen patients (35.9%) out of 39 patients showed associated injury. Among the associated injuries, rib fractures were most common findings (56.5%). Clavicle contusion, lateral condyle fracture, pubic bone fractures were seen. Four patients showed knee joint arthritis, which may affect the comfortable walking. Three patients showed shoulder joint arthritis.
Conclusion
Authors estimated the prevalence of associated injury in osteoporotic compression fracture (35.8%). Especially rib fractures or contusion occupied most portions, which is important factor in prone position and hammering of needle in processing the vertebroplasty and kyphoplasty. We concluded that we most aware of possibility of associated injury and, in that matter, 99mTc-MDP bone scintigraphy is good modality to detect associated injury of osteoporotic compression fracture.
