Effect of the Regional Bone cement Deposition Related Bone Marrow Edema on MR after Percutaneous Vertebroplasty: Pain Relief and Further Collapse of Vertebral Body

Abstract
Objective
The purpose of this study is to identify the degree of postoperative pain relief and the frequency of further collapsed vertebral body according to the percentage of bone marrow edema (BME) lesion filling with bone cement into the vertebral body.
Methods
Seventy seven patients who satisfied inclusion criteria of this study were categorized into 4 groups according to the percentage of BME lesion filling with bone cement into the vertebral body; Group A (complete): 75-100%, Group B (marked): 50-74%, Group C (mild): 25-49%, Group D (absent): 0-24%. And the degree of pain relief and the further collapsed vertebral body were measured with visual analogue scale (VAS) score, Oswestry disability index (ODI) and vertebral body compression ratio of simple X-ray.
Results
The mean postoperative VAS score improvements were as follows: group A; 5.78, group B; 5.75, group C; 5.79, and group D; 5.81. The mean postoperative ODI improvements were as follows: group A; 40.90%, group B; 41.73%, group C%; 40.80% and group D; 40.89%. As a result of the above, no statistically significant difference was found in the 4 groups (VAS, p=0.98, ODI, p=0.98). The total number of further collapses of vertebral body were 11 (14.3%) and by each group, the figures were; group A: 2 (10.5%), group B: 5 (14.7%), group C: 2 (13.3%) and group D: 2 (22.2%), presenting no statistically significant difference (p=0.87).
Conclusion
Pain relief after percutaneous vertebroplasty (PV) can occur despite insufficient BME lesion filling with bone cement injected into the vertebral body. Further collapsed vertebral body also occurs due to diverse other factors regardless of lesion filling.
